Parent Questionnaire

Does your child have any allergies, medical conditions, dietary requirements or medication we need to be aware of?
Does your child have any additional needs or require extra support?

Toileting & nappy change consent

Do you give permission for a member of our crèche team to support your child with toileting and/or nappy changes if required?

Photo & social media consent

Do you give permission for photos and videos of your child to be used on our social media?

If you select no, we will only take non-identifiable photos, such as hands or photos taken from behind where your child’s face cannot be seen.

Consent & declaration

All of our staff are DBS checked, level 3 safeguarding trained and hold a paediatric first aid certificate.
